Conservatory of Hope, Amber Galbraith, and Heath Lewis Agreed to Be Excluded for 35 Years for Allegedly Submitting False Claims

On December 17, 2024, Amber Galbraith, Heath Lewis, and Conservatory of Hope Treatment Services LLC d/b/a Conservatory of Hope Treatment Homes (collectively, “the Conservatory of Hope Parties”), Nevada, agreed to be excluded for a period of 35 years under 42 U.S.C. 1320a-7(b)(7). OIG's investigation revealed and OIG alleged that the Conservatory of Hope Parties: (1) submitted claims for services that were rendered by unqualified providers; (2) submitted claims for services that lacked supporting documentation; and (3) submitted claims for services that were not provided as claimed. Senior Counsel Gregory Becker and Associate Counsel Rebecca Lilly, with the assistance of Program Analyst Mariel Filtz, represented OIG.
